Add and delete patients

Add a patient by hand, through an intake link, a questionnaire, an uploaded form or a bulk import, and delete a patient record.

Where to find it: Sidebar → Patients → Add patient

Open this in the portal →

Steps

  1. Open Patients in the sidebar. Press n (outside a text field) to jump straight to adding a patient.
  2. Choose Add patient, then pick Manual, Intake, Upload form or Bulk import. Under Intake, choose Standard MyDosha Intake Chatbot or Own Questionnaire.
  3. Manual: fill in the form. Anything about the patient that has no field of its own goes in “Other intake notes”, which then appears under Intake.
  4. For Standard MyDosha Intake Chatbot, copy the link or send an invitation. Sending an invitation alone creates no record. A record for the resumable intake is created after the patient enters their identity and consents to begin.
  5. For Own Questionnaire, select a published questionnaire and send it to the patient or choose Complete together. A new patient's record is created when they submit the sent questionnaire, or when you start completing it together.
  6. To put a patient away, archive them: the record is kept and found under the Archived filter. A practitioner or the owner can archive; only the owner can restore it. To delete permanently, open the record and choose Delete patient. The owner gives a reason; the confirmation lists what is removed and says it cannot be undone; type the patient's full name and your password, then confirm.

Good to know

Deleting is permanent, so check your own retention obligations first. If a patient asks for deletion from their own portal and your practice keeps records under legal retention, the request is recorded instead of deleted: the patient shows a “Deletion requested” badge until you decide.
